Job summary

Oak Street Health in Newark, NJ is seeking a Nurse Practitioner to deliver effective, value-based primary care for adults on Medicare. You will work with a collaborative team to provide longitudinal care, build relationships, and ensure patients stay healthy and out of the hospital.

You will practice in outpatient clinics on a collaborative team, coordinating with physicians, PAs, and care managers to focus on primary care, care coordination, and population health.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in Family Nurse Practitioner, AGPCNP, Adult NP, or Gerontological NP.
• Active, non-probationary state NP license.
• Active DEA license.
• US Work Authorization.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the Care Team.
• Assisting the care team with phone triage and outreach.
• Educating patients on their health conditions, care plans, and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties, as assigned.
