This role focuses on developing clinical plans for patients with chronic conditions and providing care both in-clinic and remotely. Ideal candidates will have a Master’s in Nursing and experience in Primary Care.

Qualifications

• Master’s degree in nursing required.
• Board Certification as an Adult, Adult-Geriatric or Family Nurse Practitioner required.
• 6+ months nurse practitioner experience in Primary Care/Internal Medicine.
• Basic Life Support (BLS) certification is required.
• Adaptability to thrive in a fast-paced growth environment.

Responsibilities

• Develop and implement clinical plans of care for patients facing chronic conditions.
• Gather patient history and perform physical exams.
• Collaborate with the interdisciplinary team to meet patient needs.
• Educate patients and families about medication usage and medical adherence.
• Participate in continuing education as required by the state Board of Nursing.
