Job summary

Your Health in South Carolina is seeking a qualified Nurse Practitioner to service patients in the Grandstrand area. In a clinic setting, you will evaluate high-risk patients, coordinate care plans, and provide care management to improve outcomes.

The Provider role performs visits at Singleton Ridge clinic daily. This is a full-time salary-based position with a monthly bonus opportunity and a 4/10 schedule (Mon-Thu).

Qualifications

• Active Nurse Practitioner license in South Carolina.
• Experience delivering patient care in clinic/home health settings.

Responsibilities

You are an integral part of the care team, collaborating with multiple healthcare professionals to ensure comprehensive and effective patient management. Your responsibilities include evaluating high-risk patients, coordinating care plans, and providing care management to improve patient outcomes.

Our Provider role performs visits in Singleton Ridge clinic daily. This is a full-time salary-based position, with the opportunity to receive a monthly bonus.

Your Health is currently looking for a Nurse Practitioner to join our growing primary care family. A successful Provider will be able to perform these essential duties and responsibilities. Areas of Responsibility:

• Evaluate high-risk patients with complex medical needs and develop individualized care plans.
• Coordinate care activities with other members of the interdisciplinary team to ensure continuity of care and optimal patient outcomes.
• Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members to coordinate patient care.
• Participate in interdisciplinary team meetings to discuss patient progress, treatment plans, and care coordination efforts.
• Foster effective communication and collaboration to facilitate seamless patient management and interdisciplinary teamwork.
• Meet staff efficiency expectations while providing high-quality care to patients in various settings, including homes, facilities, telehealth, clinics, and specialties.
• Document care management activities accurately and efficiently to support patient care plans and regulatory compliance.
• Provide patients with education and resources related to their conditions, treatment plans, self-management strategies, and preventive care measures.
• Offer support and guidance to patients and their families to enhance their understanding of healthcare options and promote active participation in their care.
• Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.
• Monitor quality measures and gaps in care, identifying opportunities for improvement and implementing corrective actions as needed

Qualifications:

• Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required. License must be in good standing.
• Demonstrated experience in providing patient care, preferably in a home health, clinic, facility, telehealth, or specialty care setting.
• Strong communication, collaboration, and interpersonal skills, with the ability to work effectively within an interdisciplinary team.
• Ability to evaluate high-risk patients, develop care plans, and coordinate care activities to meet patient needs and improve outcomes.
• Commitment to delivering compassionate, patient-centered care and promoting a culture of excellence in healthcare delivery.
• Strong organizational and time management skills, with the ability to multitask and prioritize responsibilities effectively.
• Ability to read and communicate effectively.
• Strong written and verbal skills.
• Basic computer knowledge.
• Ability to manage and demonstrate effective leadership skills.
• Should demonstrate good interpersonal and communication skills under all conditions and circumstances.

• Must be able to work flexible hours and travel between offices, facilities, etc. Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.
