Carespring in Cincinnati is seeking an experienced Nurse Practitioner or Physician Assistant for full-time, on-site support in transitional care coordination. You will evaluate patients before admission, provide medical oversight, educate on medication management and disease care plans, and coordinate with physicians, nurses, and case managers to ensure smooth transitions.

Hours roughly 8:00am–5:00pm, Mon–Fri, with some day-to-day flexibility; on-call not routinely required.

Competenze

• Licensed Nurse Practitioner in Kentucky or Ohio.

Mansioni

• Interact clinically with physicians, NP/PAs, case managers and others to understand patient needs in transitional care.
• Evaluate candidates for transitional care management and select those best fitting the model of care.
• Conduct medication reconciliation and educate patients and caregivers on medications and plans.
• Coordinate transition of care with PCPs and other team members.
• Support process improvement to optimize the care model.
