The Carpenter Health Network, via its Homedica House Calls division, seeks a compassionate Nurse Practitioner to deliver comprehensive primary care across in-home visits, facilities, and telehealth. This full-time role supports Mississippi and Louisiana patients, emphasizing patient-centered care and collaboration with care teams.

Qualifications

• Active Nurse Practitioner license in Louisiana.
• DEA license required.
• Experience in home health, hospice, or facility-based care preferred.
• Strong communication, decision-making, and clinical skills.
• Comfortable working independently across diverse care environments.

Responsibilities

• Provide primary medical care services in home, facility, and telehealth settings.
• Conduct physical exams, wellness visits, and manage chronic and acute conditions.
• Order and interpret diagnostic tests and labs.
• Prescribe and manage medications and individualized care plans.
• Coordinate care with interdisciplinary teams and community partners.
• Promote preventive care, patient education, and continuity of care.
• Participate in after-hours or on-call rotation as needed.
