Job summary

The Carpenter Health Network is seeking a compassionate and skilled Nurse Practitioner to deliver comprehensive primary care across in-home visits, facilities, and telehealth settings through Homedica House Calls in Mississippi and Louisiana regions.

You will provide patient-centered medical care, perform exams, manage chronic conditions, and coordinate with teams to ensure continuity of care, while enjoying ESOP benefits and competitive compensation.

Responsibilities

• Provide primary medical care services in home, facility, and telehealth settings.
• Conduct physical exams, wellness visits, and manage chronic and acute conditions.
• Order and interpret diagnostic tests and labs.
• Prescribe and manage medications and individualized care plans.
• Coordinate care with interdisciplinary teams and community partners.
• Promote preventive care, patient education, and continuity of care.
• Participate in after-hours or on-call rotation as needed.

• DEA license required
• Experience in home health, hospice, or facility-based care required
• Strong communication, decision-making, and clinical skills
