Job summary

Housecall Providers is seeking an experienced Nurse Practitioner to deliver medical care and management for patients with chronic conditions, primarily in home and community settings. You will collaborate with a care team, provide comprehensive assessments, and guide care plans emphasizing palliative goals.

Required are an Oregon NP license, DEA, CPR, and 2+ years in clinical practice. The role offers a hybrid schedule with travel across Marion/Polk counties and a strong total rewards package.

Qualifications

• Master’s degree from an accredited advanced nurse practitioner program and Oregon NP license.
• DEA license and CPR certification required at hire or within 6 months.
• Valid driver's license and driving record or access to insured vehicle.

Responsibilities

• Provide diagnosis, treatment, consultation, and follow-up care for chronic and long-term conditions.
• Perform health assessments, referrals, and build proactive care plans with the care team.
• Order tests, medications, and some procedures under certification; manage refills and home health orders.
• Educate patients and families on preventive, chronic, and palliative care goals.
• Maintain records per agency protocols and participate in quality improvement.
