Circadia Health is seeking an on-site Nurse Practitioner to transform bedside care with AI-driven insights. You will see patients first, respond to predictive health signals in real time, and coach facility teams to turn data into earlier interventions, reducing readmissions.

In this role, you’ll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments across SNFs within a 30–40 mile radius. English required.

Qualifications

• AZ-licensed NP with ANCC cert for Adult/Family/Geriatric/Acute.
• 2+ years NP experience with complex adults.
• Eligible for Medicare participation; company bills on your behalf.
• Willingness to travel locally and cover 30-40 mile radius.

Responsibilities

• Conduct initial and follow-up assessments; adjust care plans.

• Interpret trend insights and initiate timely interventions.

• Perform high-volume rounds at SNFs (30+ visits/day when possible).

About the Role

Transform bedside care with AI. As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands‑on training, live demos, and shadowed assessments.

What You’ll Do

• Direct patient care: Conduct initial and follow‑up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech -> bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend-based insights, prioritize high‑risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day
• High-volume rounding: Efficiently complete visits across designated SNFs (typ. 30+/day where appropriate), focusing on new admits, facility‑flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.
• Regional flexibility: Cover sites within a 30-40-mile radius; for sites outside of that radius, mileage and lodging covered per policy.

How Success is Measured (KPIs)

• Visits: Target 30 patient visits/day
• Engagement: >=80% Time to Acknowledgement Rate: <24h
• Documentation: Complete all notes using AI‑powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

Must-Have Qualifications

• Certified and licensed Nurse Practitioner in AZ; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid AZ driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required

• Tech‑savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
• Spanish is preferred.
