Job summary

Your Health Organization in South Carolina seeks a Nurse Practitioner to deliver primary care and acute visits across homes, ALFs, and ILFs within assigned service areas. The NP will assess, diagnose, and treat patients in collaboration with physicians, developing and implementing treatment plans and coordinating referrals as needed.

The position is salary-based, full-time with shifts including 12-hour and 8-hour options, and offers a monthly bonus.

Qualifications

• Master's or Doctorate degree in Nursing from an accredited program.
• Current SC NP licensure.
• Certification as NP in a specialty area (e.g., Family, Adult‑Gerontology, Pediatrics).
• 2 years of NP experience preferred but not required.
• Strong clinical assessment and diagnostic skills.
• Proficiency in EMR systems and healthcare tech.
• Commitment to professional development.
• CPR certification and related certifications as required.

Responsibilities

• Provide advanced nursing care within scope of practice.
• Assess, diagnose, and treat patients in various settings (homes, clinics, etc.).
• Develop and implement treatment plans including medications and therapies.
• Educate patients and families on health conditions and preventive care.
• Coordinate referrals and collaborate with healthcare team.

This is a salary-based position, with the opportunity to receive a monthly bonus. This is a full-time with the following schedule(s) and service area(s) available.

The following shift(s) are available:

• 12-hour shift (working 7 days on 7 days off)
• 8-hour shift (Monday-Friday; 8:00am-5:00pm)

What Are We Looking For?

Your Health is currently looking for a Nurse Practitioner to join our growing urgent care family. As a Nurse Practitioner, you will provide advanced nursing care to patients within your scope of practice. You will work closely with physicians and other healthcare professionals to assess, diagnose, and treat patients in various settings, such as hospitals, clinics, or private practices. A successful Nurse Practitioner should be able to conduct comprehensive patient assessments, including medical history, physical examinations, and diagnostic tests. Develop and implement treatment plans, including prescribing medications, therapies, and other interventions. Monitor patient progress and adjust treatment plans as necessary. Educate patients and their families about health conditions, treatments, and preventive care. Collaborate with other healthcare providers to coordinate patient care and referrals. Maintain accurate and up-to-date patient records and documentation.

Qualifications

• Master's or Doctorate degree in Nursing from an accredited program.
• Current licensure as a Nurse Practitioner in SC
• Certification as a Nurse Practitioner in [specialty area] (e.g., Family, Adult-Gerontology, Pediatrics, etc.).
• 2 years of experience working as a Nurse Practitioner preferred but not required.
• Excellent clinical assessment and diagnostic skills.
• Strong interpersonal and communication skills.

• Proficiency in electronic medical record (EMR) systems and other healthcare technology.
