Job summary

Circadia Health is seeking a hands‑on Nurse Practitioner to join our embedded care model in Sacramento. You will deliver direct patient care, perform initial and follow‑up assessments, and drive real‑time interventions based on predictive health signals.

You will train facility staff on Circadia workflows, interpret trend data, and ensure timely documentation. The role combines patient care with tech integration in a startup environment.

Qualifications

• Tech‑to‑bedside integration: train staff to integrate Circadia into clinical workflows
• Data into action: interpret trend‑based insights and prioritize high‑risk patients for timely interventions
• Documentation: complete notes and charts within the same day
• High‑volume rounding: conduct visits across SNFs, focusing on new admits and flagged patients
• Mentor & collaborate: support NP onboarding and partner with clinical ops/product teams

As Circadia’s on‑site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech‑to‑bedside integration through hands‑on training, live demos, and shadowed assessments.

This is a full‑time, embedded NP role requiring Monday‑Friday, on‑site presence at our partner facility in Sacramento, CA.

Key Responsibilities

• Direct patient care: Conduct initial and follow‑up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech to bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend‑based insights, prioritize high‑risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day
• High‑volume rounding: Efficiently complete visits across designated SNFs (typ. 30+/day where appropriate), focusing on new admits, facility‑flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams

Performance Metrics

• Visits: Target 30 patient visits/day
• Engagement: ≥80% Time to Acknowledgement Rate: < 24h
• Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

Qualifications

• Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid CA driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required

Preferred Qualifications

• Tech‑savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
• Spanish is preferred.
