Job summary

Phoenix House of New York, Long Island City, seeks a Nurse Practitioner specialized in psychiatry to deliver evidence-based care to patients with psychiatric diagnoses and substance use disorders.

You will conduct comprehensive assessments, manage medications, and collaborate with a multidisciplinary team to promote recovery and continuity of care, including on-call coverage as needed.

Qualifications

  • Master's or Doctoral degree in an approved NP/PA program.
  • BA or BS.
  • New York State AGNP, FNP, or PMHNP and RN licensure.
  • Nurse Practitioner Board Certification; ACLS/BLS active.

Responsibilities

  • Conduct comprehensive assessments of acute psychiatric and SUD conditions.
  • Maintain an ongoing case panel for assessment, meds, treatment planning, and follow-up.
  • Provide cross-coverage for continuity of care as needed.
  • Respond to acute clinical needs including suicidality, withdrawal, or psychosis.
  • Review medical/psychiatric history and ensure timely documentation.
  • Coordinate with pharmacy and outside providers to promote continuity of care.
  • Provide on-call weekend/holiday coverage.

You Will Have Responsibilities Including But Not Limited To
  • Conduct a timely comprehensive assessment of acute psychiatric and substance use disorder related conditions.
  • Maintain an individual case panel providing ongoing psychiatric, and substance use disorder assessment, medication management, treatment planning, and follow up.
  • Provide cross-coverage for clients assigned to other providers as needed for continuity of care, and timely access to care.
  • Assess and respond to acute clinical needs including but not limited to suicidal or homicidal ideation, self-harm risk, mania, psychosis, severe anxiety, withdrawal, intoxication, substance cravings or significant changes to mental status or presentation within the scope of care practice while engaging with on-call medical director, nursing leadership and other supports.
  • Review past medical, psychiatric and substance use history of new admissions and assess for urgent/emergent conditions that need immediate attention.
  • Maintain thorough and accurate and timely documentation of clinical encounters including clinical decision making. Completion of notes within 48 hours of client encounter – sooner based on acuity.
  • Document all encounters with client and or staff related to client conditions or concerns in the medical record.
  • Participates in interdisciplinary care and case conferencing, working closely with nursing, counselors/CASACs, licensed mental health clinicians, recovery support specialists (RSS), peers, medical providers, and other team members.
  • Coordinate with pharmacy, medical team, and outside providers as needed to promote continuity of care.
  • Provide on call weekend and holiday coverage as assigned with timely responses to clinical questions during coverage.
  • May be asked to provide emergency/urgent coverage at alternative Phoenix House locations based on need either in person or virtually based on organizational or client need.
  • Ensure timely follow up for clients seen in the ED or admitted to the hospital to promote safe transitions of care.

Qualifications
REQUIRED EDUCATION
  • Successful completion of a Master's or Doctoral from an approved nurse practitioner or physician assistant program.
  • BA or BS
Required Licenses/Certifications
  • New York State AGNP, FNP, or PMHNP and RN licensure
  • Nurse Practitioner Board Certification
  • Must be currently licensed as a registered nurse in New York state.
  • NPI Number
  • DEA License
  • Medicaid / Medicare Number
  • Active ACLS/BLS certification
Required Experience
  • Qualified by training, education, and experience in providing medical care and assessment, in collaboration with a physician, to clients with substance use disorders experiencing dependence and withdrawal
  • Three plus years related experience preferred

WORK ENVIRONMENT
  • This job operates in an indoor medical clinic environment. This role also routinely meets patients who may have contagious illnesses. Appropriate PPE will be provided and required.
