Overview

Great side hustle. Get out of the Hospital!!! Visits scheduled around your availability! The Nurse Practitioner (NP) brings expert care to members, through clinical assessments, and screenings. This role may provide care in a variety of settings including member/participant homes, assisted living facilities, and skilled nursing facilities. There are PRN and Full Time Opportunities available!

Responsibilities
  • Perform screenings including medical history, physical exam, medication reconciliation and education.
  • Effectively identify, evaluate, and address the concerns of the member, employee or participant. Order and interpret diagnostic imaging, laboratory results and provide clinically appropriate interventions, additional tests, or referral as needed.
  • Administer and order point of care and lab-based testing.
  • Refer members to PCP, case management, disease management programs, and/or appropriate specialist as necessary.
  • Work collaboratively with a variety of stakeholders including risk & quality teams, staffing, operations, physicians, case managers, social workers, care givers and ancillary medical personnel.
  • Participate in all mandatory compliance and other training programs, seek guidance for compliance-related concerns and adhere to all applicable laws, regulations and Matrix policies and procedures.
  • Create and maintain clear documentation according to clinical standards, guidelines and protocols.
  • Maintain positive control of all PHI information and compliance with OSHA and State Board of Nursing. May act as a preceptor for new providers. Other duties may be assigned.
Position Requirements
  • Educational Requirements: Bachelor of Science in Nursing preferred. Master’s Degree required OR commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority.
  • Board certification by AANP, AACN or ANCC, in Adult, Family Practice Geriatrics or Adult-Gerontology Primary Care, or ACNPC-AG (Adult Gero) Acute Care NP, or ACNPC (Adult) Acute Care NP.
  • Current CPR certification. Multistate licensure preferred.
  • Knowledge, Skills and Abilities:
  • Demonstrated ability to: Perform comprehensive geriatric, adult physical exams, based on scope of practice for licensure and certification.
  • Possess a valid state driver license and proof of coverage and meet company’s driving and automobile liability insurance standards.
  • Ability to travel to assigned locations via public transportation including airplanes. Proficient computer skills, including e-mail and Microsoft Office.
  • Effective written communication skills including but not limited to writing reports and correspondence. Strong verbal communication and effective listening skills, including the ability to present and maintain a professional, empathetic demeanor at all times.
  • Demonstrated ability to prioritize work, meet deadlines, and use time efficiently.
