Job summary

Bristol Hospice is seeking a compassionate Nurse Practitioner to join our hospice team in Burr Ridge, IL. This PRN role supports terminally ill patients and their families, delivering high-quality, patient-centered care within the interdisciplinary team.

Responsibilities include medical management, care planning, and coordination of hospice services, with a focus on comfort, dignity and respectful end-of-life care. PRN Monday–Friday 8am–5pm with some flexibility.

Qualifications

  • Graduate of an accredited School of Nursing and NP program.
  • Current state's nursing licensure.
  • Master's degree with a minimum of one year NP experience and one year hospice/palliative care experience.
  • Certification in a specialist area preferred (CHPN, Pediatrics or Geriatrics).
  • Must have a valid driver's license and insured vehicle as required by policy.

Responsibilities

  • Provide medical care, palliate and manage terminal illness under supervision of a collaborating physician for adults and children.
  • Complete history and physical examinations and document findings in the medical record.
  • Develop individualized care plans, prescribe medications, coordinate hospice services and obtain consultations.
  • Ensure hospice eligibility assessments and complete face-to-face visit requirements as required.
  • Collaborate with the hospice medical director and interdisciplinary team to optimize patient care.
  • Support end-of-life counseling and family communication as part of care.

QualificationsOn an Average Day You Will:

(includes, but not limited to)

  • Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children
  • Complete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record
  • Manage health conditions and terminal illness by developing an individualized plan ofcare, prescribing medications and/or treatments, obtaining consultations or makingreferrals and coordinating hospice care services
  • Complete the hospice face to face visit requirements prior to the third and subsequenthospice benefit periods to assess hospice eligibility based on the hospice conditions ofparticipation and the local coverage determinations
  • Provide clinical information to the hospice medical director for interpretation of eligibilityfor hospice recertification
  • Make recommendations to the hospice medical director regarding patient eligibility forcertification and recertification for hospice care
  • Review clinical information, coordinate and oversee patient care in consultation with thehospice interdisciplinary team and other healthcare practitioners
  • Assure that patient receives appropriate measures to control symptoms, throughcollaboration with interdisciplinary team members
  • Review and develop protocols for treatment and propose options for interventionsbased on the literature in collaboration with the collaborating physician
  • Consult the collaborating physician when the patient's plan of care is outsidestandardized practice and protocols
  • Collaborate with the interdisciplinary group to create, review and revise the patient'splan of care
  • Fulfill the obligation of requested and/or accepted assignments
  • Demonstrate knowledge in communication and counseling patient/family in dealing withend-of-life issues
  • Complete all other duties as assigned
Requirements:
  • Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program
  • Must have current nursing licensure in State
  • Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience
  • Must have accreditation from an approved certifying body for advanced practice nursing as required by the State
  • Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
  • Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order
