Job summary

Bristol Hospice is seeking a compassionate Nurse Practitioner to provide hospice medical care, manage terminal illnesses, and work within an interdisciplinary team to support patients and families.

The role requires an accredited NP program, current state licensure, and a Master’s degree with hospice experience. Certification in a hospice specialty is preferred. A valid driver’s license is required, and you will join a mission-driven organization dedicated to compassionate end-of-life care.

Qualifications

• Graduate of an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current nursing licensure in the state.
• Master's degree with a minimum of one year Nurse Practitioner experience and at least one year hospice or palliative care experience.
• Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing - CHPN).
• Must be a licensed driver with a valid automobile and valid insurance as required by the organization/state.

Responsibilities

• Provide medical care and manage terminal illness in adults and children under the supervision of a collaborating physician.
• Complete history and physical examinations and interpret diagnostics; document findings.
• Develop individualized care plans, prescribe medications, obtain consultations, and coordinate hospice care services.
• Complete hospice face-to-face visit requirements to assess eligibility for hospice periods.
• Provide information to the hospice medical director for eligibility reviews and recertification decisions.
• Coordinate patient care with the hospice interdisciplinary team and other practitioners.
• Review and develop treatment protocols in collaboration with the collaborating physician.
• Collaborate with the interdisciplinary group to update patient plans of care.
