Qualifications

• Bachelor’s degree required.
• Nurse Practitioner program registered by NYS Education Department.
• Master’s degree required.
• Current NYS RN license.
• Current NYS NP license.

Responsibilities

• Obtains and documents patient history relevant to chief complaint or reason for visit.
• Performs relevant physical exam and documents findings.
• Diagnoses, treats diseases and manages chronic illness.
• Prescribes medications in accordance with NYS and federal law.
• Orders appropriate laboratory, radiological and special examinations or tests; interprets results.
• Provides preventative health services, including health education and counseling.
• Collaborates with other clinicians and staff to ensure continuity of care.
• Performs other job-related duties as required.

Summary Of Position

Principal Accountabilities

• Obtains and documents patient history as appropriate to chief complaint or reason for visit.
• Conducts relevant physical exam and evaluates patient, documenting pertinent positive and negative findings.
• Diagnoses and treats disease and manages chronic illness.
• Prescribes medication in accordance with New York State and federal law.
• Orders appropriate laboratory, radiological and special examinations or tests; interprets results
• Provides preventative health services, including health education and counseling.
• Collaborates with other clinicians and staff members to ensure continuity of care of patients.
• Performs other job-related duties as required.
