Job summary

AdvantageCare Physicians on the Upper West Side of Manhattan seeks a Nurse Practitioner to provide primary care services in collaboration with physicians. You will evaluate patients, diagnose, and manage acute and chronic conditions within a multidisciplinary team.

Responsibilities include patient history, physical exams, ordering tests, prescribing medications in accordance with NYS regulations, and delivering preventive care and patient education to diverse populations.

Qualifications

• Bachelor's Degree (R)
• Graduate of a Nurse Practitioner education program registered by the NYS Education Department (R)
• Master's Degree (R)
• Licensed and currently registered as a Registered Nurse by NYS
• Current New York State Registration as Nurse Practitioner
• Current Board Certification as an NP from a NYS approved accrediting body

Responsibilities

• Obtains and documents patient history as appropriate to chief complaint or reason for visit.
• Conducts relevant physical exam and evaluates patient, documenting pertinent positive and negative findings.
• Diagnoses and treats disease and manages chronic illness.
• Prescribes medication in accordance with New York State and federal law.
• Orders appropriate laboratory, radiological and special examinations or tests; interprets results
• Provides preventative health services, including health education and counseling.
• Collaborates with other clinicians and staff members to ensure continuity of care of patients.
• Performs other job‑related duties as required.
