Job summary

Oak Street Health is hiring a Nurse Practitioner to deliver value-based primary care for adults on Medicare in outpatient clinics. You will work within a collaborative care team that includes physicians, PAs, medical assistants, and care managers to coordinate comprehensive health plans.

Ideal candidates hold MSN/DNP/NP certification with national NP credentials, an active NP license, and a DEA license, along with US work authorization.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program.
• National certification in one of the specified NP specialties.
• Active, non-probationary NP license and DEA registration.
• US work authorization is required.

Responsibilities

• Provide effective and equitable value-based primary care to adults on Medicare.
• See patients independently and collaborate with physicians as needed.
• Deliver longitudinal care, educate patients and coordinate with care team.
• Engage in population health focus and care coordination across settings.
• Support outreach, home visits, and participation in clinic/quality initiatives.
