Job summary

Native American Health Center, Inc. (NAHC) in Oakland, CA seeks a Nurse Practitioner to deliver comprehensive primary, preventive, chronic and urgent care as part of a multi-disciplinary team.

The role reports to the Medical Director and supports the Medical Department in a full-time, 32-40 hour/week capacity. Applicants must hold a current RN license, an FNP certificate with furnishing number, and demonstrate strong communication and teamwork.

Qualifications

• Current RN license and FNP certification with furnishing number.
• Strong clinical knowledge in primary care and chronic disease management.
• Excellent communication and teamwork skills.

Responsibilities

• Provide primary care to members, including histories, exams, labs, prescriptions and referrals.
• Document visits in EHR within 72 hours.
• Coordinate care with internal and external providers to ensure high quality service.
• Educate patients and participate in quality improvement activities.
• Collaborate with the multidisciplinary team to deliver integrated care.
