Job summary

Native American Health Center (NAHC) invites applications for a Nurse Practitioner to provide comprehensive primary and preventive care as part of a multidisciplinary team at the Oakland clinic. You will manage patient histories, exams, prescriptions, and referrals, while documenting all visits in the EHR within 72 hours.

The role emphasizes collaborative care with nutritionists, social workers, and other clinicians, adherence to NAHC visit models, and active participation in quality improvement

Qualifications

• Current RN license and FNP certificate are required.
• Current CPR certification required.
• Furnishing number required.
• One year of experience in a community clinic preferred.
• Ability to work independently and in a multi-disciplinary team.

Responsibilities

• Provide primary care including histories, exams, labs, prescriptions and referrals.
• Document visits accurately in the EHR within 72 hours.
• Coordinate care with internal and external providers and hospitals.
• Participate in quality improvement and panel management.
• Educate medical team members and contribute to trainings.
• Support school-based clinics with outreach and case management if assigned.
