As a Nurse Practitioner in our Idaho Falls/Ammon clinic, you will deliver high-touch, coordinated care to a panel of senior patients with support from MAs/LPNs, front office, clinic manager, and medical director.

Qualifications

• Active Idaho Nurse Practitioner license.
• Practice in Primary Care and independent patient care 3–5+ years (Required).
• Nurse Practitioner certification from ANCC/AANP.
• Prescriptive Authority by the ID state board and DEA license.
• CPR/BLS certification and current Idaho driver's license.

Responsibilities

• Provide full scope of primary care services including well, acute, and chronic condition management.
• Take medical histories, examine patients, diagnose, and prescribe within state guidelines.
• Perform Wellness Visits and Health Risk Assessments with accurate clinical coding and documentation.
• Conduct visits in clinic, telehealth, and home visits as needed.
• Manage patient panels and coordinate care with the care team for high-quality outcomes.
• Order tests, interpret results, and provide preventive education to patients.
