Job summary

Protouch Staffing is seeking a Nurse Practitioner to deliver comprehensive primary medical care to frail and elderly participants in Anaheim, CA. You will work under the Medical Director with the IDT to perform assessments, diagnose, treat, and plan care.

Responsibilities include chronic disease management, medication management, testing, referrals, and occasional home visits. Requires CA NP license, DEA, BLS/CPR, and 3–5 years of nursing experience in geriatric primary care.

Qualifications

  • Graduate of an accredited Nurse Practitioner program.
  • Current California Nurse Practitioner license required.
  • DEA registration required.
  • Current BLS/CPR and First Aid certification required.
  • Minimum of 3-5 years of nursing experience, including at least 1 year with frail/elderly populations.
  • Experience in geriatric primary care and interdisciplinary teams preferred.

Responsibilities

  • Provide comprehensive primary care, including assessment, diagnosis, treatment, preventive care, and management of acute and chronic conditions.
  • Conduct initial and quarterly medical assessments, including histories and physical exams.
  • Develop and monitor individualized Plans of Care.
  • Diagnose and treat episodic illnesses and manage complex frail elderly patients.
  • Oversee clinic operations and coordinate referrals with specialists.
  • Order and interpret diagnostic tests, including labs and EKGs.
  • Prescribe medications per California NP authorities.
  • Provide home visits and coordinate care across settings.
  • Educate patients and families on medications and treatment plans.

Job Summary

Under the supervision of the Medical Director and in collaboration with the interdisciplinary team (IDT), the Nurse Practitioner provides comprehensive primary medical care to participants of Innovative Integrated Health. Responsibilities include initial and ongoing participant assessments, diagnosis and treatment, care planning, chronic disease management, medication management, and coordination of care for frail and elderly populations.

Roles & Responsibilities
  • Provide comprehensive primary care, including assessment, diagnosis, treatment, preventive care, and management of acute and chronic conditions.
  • Conduct initial and quarterly medical assessments, including medical histories and physical examinations.
  • Develop and monitor individualized Plans of Care.
  • Diagnose and treat episodic illnesses and manage complex conditions affecting frail and elderly populations.
  • Oversee the management and clinical operations of the primary care clinic.
  • Order, perform, and interpret diagnostic tests, including laboratory studies and EKGs.
  • Prescribe medications and treatments within California NP prescriptive authority.
  • Perform injections, immunizations, EKGs, and other therapeutic procedures as clinically appropriate.
  • Coordinate referrals and collaborate with medical specialists.
  • Provide clinical oversight for hospitalized participants and coordinate hospital discharge planning.
  • Provide primary care through clinic visits, telephone consultations, nursing facility coordination, and home visits as needed.
  • Collaborate with the interdisciplinary team, caregivers, and families.
  • Participate in IDT meetings, case conferences, and other required activities.
  • Educate participants and families regarding medications, treatment plans, disease management, and preventive care.
  • Maintain accurate and complete medical documentation.
  • Participate in QAPI and clinical quality improvement programs.
  • Make home visits as needed.
  • Participate in staff meetings, training, committees, and organizational projects.
  • Comply with organizational policies, healthcare regulations, safety procedures, and applicable California laws and professional standards.
  • Perform other related duties as assigned.
  • May require use of a personal vehicle for participant-related services or home visits.
Qualifications & Experience
  • Graduate of an accredited Nurse Practitioner program.
  • Current California Nurse Practitioner license required.
  • DEA registration required.
  • Current BLS/CPR and First Aid certification required.
  • Minimum of 3-5 years of nursing experience, including at least 1 year of experience working with frail or elderly populations required.
  • Experience providing primary care to geriatric or medically complex patients strongly preferred.
  • Experience working within an interdisciplinary team, PACE program, community clinic, or similar integrated care setting preferred.
  • Supervisory, clinical leadership, or mentoring experience preferred.
  • Advanced clinical assessment and physical examination skills.
  • Strong knowledge of primary care, chronic disease management, geriatric medicine, medication management, and applicable California and federal healthcare regulations.
