Job summary

Contra Costa County Health Services Department seeks Nurse Practitioners to deliver medical care across the County’s hospital, ambulatory centers, and detention facilities. You will provide primary care, case management, and mental health services as assigned, working closely with physicians and the multidisciplinary team.

Requires CA RN license, NP certificate, NP Furnishing license, and a Master's or related Nursing degree.

Qualifications

• License required: RN license, NP certificate, NP Furnishing license in CA.
• Master's degree in nursing or related field; Graduate degree in nursing from accredited program.

Responsibilities

• Provide medical care to individuals and families in hospital, ambulatory, mental health, and detention facilities.
• Provide primary care and case management, and mental/behavioral health care as assigned.
• Collaborate with physicians and interdisciplinary treatment team; participate in quality improvement committees.
