Job summary

BJC Medical Group is seeking a Nurse Practitioner for Post-Acute Care to cover O'Fallon, St. Peters, Chesterfield, Lake St. Louis and Maryland Heights.

The role offers Monday–Friday days with a flexible schedule and no weekend work, with call for primary locations by phone only. Responsibilities include managing acute and chronic illnesses, performing assessments, and initiating treatment plans within ambulatory and hospital settings under the medical direction of a collaborating physician.

Qualifications

• Master's Degree in Nursing required.
• Nurse Practitioner license and RN certification required.
• Under 2 years of experience, 2–5 years preferred.

Responsibilities

• Manages health care of patient populations in Cardiology, Vascular Surgery, Orthopedics and Post-Acute in hospital, clinic and skilled nursing settings.
• Develops treatment plans and initiates care under physician direction.
• Hospitals rounds and on-call coverage for inpatient services and ambulatory follow-ups.
