Job summary

BJC Medical Group is seeking a Board Certified/Board Eligible Internal Medicine, Family Medicine, Geriatric Medicine, or Emergency Medicine physician for its Post-Acute Care program in Farmington and Park Hills, Missouri. Qualifications

• MD or DO degree required.
• Board certification or eligibility in one of the specified specialties.
• Missouri medical licensure eligible.

Responsibilities

• Provide medical care for post-acute, long-term care, and assisted living residents.
• Collaborate with nursing leadership, rehabilitation services, and care teams.
• Complete medical records, MyChart, and inbasket management.
• Participate in care transitions and quality improvement initiatives.
• Deliver patient-centered care with strong communication and teamwork.
• Maintain autonomy while collaborating with interdisciplinary staff.
