About the Position

Position: NP PM&R Clinician
Location: Bloomsburg, PA or Richfield, PA
Employment Type: Full‑time (32‑40 hours per week), W‑2
Salary: $95,000 – $132,000 plus bonus
Schedule: 8‑hour shift with flexible days; hours should hover around 9AM – 4PM

Responsibilities

• Provide specialized Physical Medicine & Rehabilitation (PM&R)/Physiatry services to SNF patients
• Represent CRC within the facility, leading clinical collaboration with the interdisciplinary care team
• Collaborate with PCP teams to manage common SNF conditions such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries
• Work alongside PT, OT, and SLP teams to improve patient function and quality of life
• Track therapy patients’ progress and assess barriers such as joint or neuropathic pain, spasticity, neurogenic bowel/bladder, and more
• Attend multidisciplinary meetings and bridge gaps between therapy and internal medicine
• Participate in culture‑building and ongoing education (monthly didactic lectures, virtual regional meetings, and regional dinners)
