Job summary

Bluestone Physician Services is seeking an on-site NP/PA provider to deliver team-based primary care within group home living communities across Minnesota, Wisconsin, and Florida. You will autonomously manage a patient panel of roughly 8–14 per day while tailoring your schedule to part-time desires and needs.

The role requires Minnesota licensure, strong communication, and excellent interpersonal skills, plus travel to Twin Cities Metro sites. Travel and local conditions apply.

Qualifications

• Graduate of NP or PA program from an accredited school.
• Must have and maintain a valid Minnesota License.
• Professional clinical experience in primary care.
• Proven ability to communicate effectively with strong verbal skills.
• Excellent interpersonal and customer service skills.
• Must have a valid driver's license and minimum state required liability auto insurance; local travel is required.
• Travel throughout Twin Cities Metro required.
• Preferred: 2+ years of clinical experience and/or group home experience.
