Job summary

Bluestone Physician Services delivers great outcomes by bringing exceptional care to patients living with complex, chronic conditions and disabilities. Our on-site, team-based primary care travels to patients in group homes across Minnesota, Wisconsin and Florida, including Roseville.

You’ll join a growing practice as NP or PA to provide proactive, preventative care in a supportive environment. Part-time providers care for 8–14 patients daily, with flexible scheduling and strong clinical support

Qualifications

• Graduate of NP or PA program from an accredited school.
• Must have and maintain a valid Minnesota License.
• Professional clinical experience in primary care.
• Strong verbal communication skills.
• Excellent interpersonal and customer service abilities.
• Valid driver's license with auto insurance; local travel required.
• Willingness to travel throughout Twin Cities Metro.

Responsibilities

• Provide on-site, team-oriented primary care in group home settings.
• Care for patients weekdays with a part-time schedule.
• Autonomously manage a patient panel of approximately 8-14 per day.
• Design and manage your schedule and care plans.
• Collaborate with LPNs, RNs and social workers for care coordination.
• Coordinate with home care, hospice and community resources.

Nurse Practitioner / Physician Assistant - Primary Care (Group Home Population)

Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing in Group Homes in Roseville and surrounding communities.

Position Overview:

• Provide on-site, team-oriented primary care to patients within the group home setting living communities
• Part-time providers care for patients weekdays; tailored to part-time desires and needs
• Manage a panel of patients autonomously seeing approximately 8-14 patients per/day
• Ability to design and manage your schedule
• Develop and implement care plans appropriate to patients and their desires
• Clinical support from clinical assistants and care coordination through LPNs, RNs and social workers
• Collaborate with community partners within home care, hospice, and other resources as needed

Qualifications:

• Graduate of NP program or PA program from an accredited school
• Must have and maintain a valid Minnesota License
• Professional clinical experience in primary care
• Proven ability to communicate effectively with strong verbal skills
• Excellent interpersonal and customer service skills
• Must have a valid driver’s license and minimum state required liability auto insurance; local travel is required
• Demonstrated compatibility with Bluestone’s mission and practiticing philosophies
• Travel throughoutTwin Cities Metro required

Preferred Qualifications:

• 2+ years of clinical experience and/or group home experience in another role
