Job summary

Bluestone Physician Services seeks a Nurse Practitioner or Physician Assistant for primary care in group-home settings in the Twin Cities metro, including Roseville, MN. The role is part-time, on-site, with a focus on delivering team-based primary care to residents in Assisted Living, Memory Care, and Group Home communities.

Providers will manage a patient panel of roughly 8-14 visits per day, design and adjust care plans, and collaborate with clinical assistants, LPNs, RNs, and social workers

Qualifications

• Graduate of NP program or PA program from an accredited school.
• Must have and maintain a Minnesota License.
• Professional clinical experience in primary care.
• Proven ability to communicate effectively with strong verbal skills.
• Excellent interpersonal and customer service skills.
• Must have a valid driver’s license and minimum state required liability auto insurance; local travel is required.
• Demonstrated compatibility with Bluestone’s mission and practiticing philosophies.
• Travel throughout Twin Cities Metro required.

Preferred Qualifications:

• 2+ years of clinical experience and/or group home experience in another role
