Job summary

LanceSoft is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join a mobile primary care team serving geriatric patients in assisted living and memory care communities across Decatur and Springfield, IL.

This autonomous role emphasizes building meaningful patient relationships, delivering high-quality, patient-centered care, and coordinating care with staff and other providers in community settings.

Qualifications

• Active Nurse Practitioner or Physician Assistant license in Illinois.
• Active DEA registration.
• Minimum of 2 years of NP/PA experience required.
• Strong clinical assessment and patient management skills.
• Experience working with geriatric patients preferred.
• Ability to work independently and manage a daily patient schedule.

Responsibilities

• Provide comprehensive primary care services to geriatric patients.
• Conduct wellness visits, chronic disease management, and preventive care.
• Assess, diagnose, and develop treatment plans for patients.
• Prescribe medications and manage ongoing care needs.
• Coordinate care with community staff, caregivers, and healthcare providers.

Compensation: $180,000 annually (base salary + incentives).

Schedule:

• Monday – Friday
• 40 hours per week
• No weekends
• No holidays
