Job summary

Brigade Health’s Access Healthcare Associates unit in California is seeking a compassionate Nurse Practitioner or Physician Assistant to join our Mobile Primary Care Team serving older adults at home and in senior living communities. You will conduct comprehensive assessments, develop care plans, and manage chronic conditions with a team that values autonomy and collaboration.

Qualifications

• Active, unencumbered NP or PA license in the state you will be practicing.
• Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).
• Compassionate, patient-centered approach and strong clinical judgment.
• Valid driver's license and reliable transportation.
