$40,000 Student Loan Repayment Or $25,000 Sign-on Bonus for Individuals Who Have Not Previously Participated in this Program

This is a travel role covering the State of Connecticut and will require overnight travel. This role comes with great incentives!

As a team member of our Senior Community Care (SCC) product, you will work with a team to provide care to patients at home, including in nursing homes and assisted living facilities for seniors. This work aims to improve access to care and support.

The SCC program is a longitudinal, integrated care delivery program that coordinates clinical care for members in facility settings. The APC within SCC provides care to our highest-risk health plan members as part of an interdisciplinary team. This role offers work-life balance with no on-call, weekends, or holidays required. The position requires travel throughout Connecticut.

Primary Responsibilities:
  • Provide clinical coverage for the primary APC during PTO or absences
  • Manage members during acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions
  • Manage members during short-term, sub-acute rehabilitation episodes
  • Conduct comprehensive, routine, and/or acute care visit assessments
  • Manage acute and chronic medical and behavioral conditions effectively
  • Ensure accurate documentation, including ICD-10 codes
  • Collaborate with the interdisciplinary care team

Required Qualifications:
  • Certified APC through a national board:
    • For NPs: Graduate of an accredited MSN program, board certified by AANP or ANCC, with preferred certifications in ANP, FNP, or GNP
    • For PAs: Graduate of an accredited Physician Assistant program, board certified by NCCPA
  • Active and unrestricted APC license in Connecticut
  • Current DEA licensure or ability to obtain post-hire
  • Ability to gain a collaborative practice agreement if applicable
  • Physical ability to perform assessments and move a 30-pound bag
  • Reliable transportation
  • At least 1 year of clinical experience, preferably in long-term care
Preferred Qualifications:
  • Experience with patients with complex behavioral, social, or functional needs
  • Knowledge of geriatrics and chronic illnesses
  • Proficient in computer documentation and electronic records

Note: Employees must be in active regular status, stay in role for at least 12 months, and remain in an APC or Physician role within Home and Community for 36 months to receive full student loan repayment benefits.

The salary range is $89,800 to $176,700 annually, based on full-time employment and various factors. Benefits include comprehensive packages, incentives, stock purchase plans, and 401(k). We are committed to equitable, inclusive care and a diverse workforce.
