$40,000 Student Loan Repayment or $25,000 Sign-on Bonus for Individuals Who Have Not Previously Participated in this Program

This is a travel role covering the state of Connecticut and will require overnight travel. It offers great incentives!

As part of our Senior Community Care (SCC) team, you will provide care to patients at home, including nursing homes and assisted living facilities, supporting improved access to care.

The SCC program is an integrated care delivery model for high-risk members in facility settings. The APC in SCC provides care to these members as part of an interdisciplinary team. This role offers work-life balance, with no on-call, weekends, or holidays required. The position requires travel throughout Connecticut.

Primary Responsibilities:
  1. Provide clinical coverage for the primary APC during PTO or absences.
  2. Manage members during acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions.
  3. Manage members during short-term, sub-acute rehabilitation episodes.
  4. Conduct comprehensive, routine, and/or acute care assessments.
  5. Manage acute and chronic medical and behavioral conditions effectively.
  6. Ensure accurate documentation, including ICD-10 coding.
  7. Collaborate with the interdisciplinary care team.
Required Qualifications:
  • Certified APC through a national board.
  • For NPs: Graduate of an accredited MSN program, board certified by AANP or ANCC, with preferred certifications as ANP, FNP, or GNP.
  • For PAs: Graduate of an accredited Physician Assistant program, certified by NCCPA.
  • Active and unrestricted APC license in Connecticut.
  • Current DEA licensure or ability to obtain post-hire.
  • Ability to gain a collaborative practice agreement if applicable.
  • Physical ability to perform assessments and move equipment.
  • Reliable transportation.
  • At least 1 year of clinical experience, preferably in long-term care.
Preferred Qualifications:
  • Experience with patients with complex behavioral, social, or functional needs.
  • Knowledge of geriatrics and chronic illnesses.
  • Proficient in electronic medical records documentation.

Note: Employees must be active in a regular role for at least 12 months to qualify for student loan repayment benefits, which require a 36-month commitment in the APC or Physician role within Home & Community.

The salary range is $89,800 to $176,700 annually, based on full-time employment, experience, and location. Benefits include comprehensive packages, incentives, equity options, and 401(k).
