This is a travel role covering the State of Connecticut and will require overnight travel. This role comes with great incentives!

The Senior Community Care (SCC) program is a longitudinal, integrated care delivery program that coordinates the clinical care of members in a facility setting. The APC within SCC provides care to our highest-risk health plan members and is part of an interdisciplinary team. This position allows for work-life balance: no on-call, no weekends, and no holidays. It requires travel through the state of Connecticut.

Primary Responsibilities:

  • Provide clinical coverage for the primary APC during PTO or absences
  • Manage members with acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions
  • Conduct comprehensive, routine, and acute care visit assessments
  • Manage acute and chronic medical and behavioral conditions effectively
  • Ensure accurate and complete documentation, including ICD-10 conditions
  • Collaborate with the interdisciplinary care team

Required Qualifications:

  • Certified APC through a national board. For NPs: Graduate of an accredited MSN program, board certified through AANP or ANCC, with preferred certifications in ANP, FNP, GNP. For PAs: Graduate of an accredited Physician Assistant program, certified by NCCPA.
  • Active and unrestricted APC license in Connecticut
  • Current DEA licensure or ability to obtain post-hire, per state regulations
  • Ability to complete physical requirements and perform physical assessments
  • Reliable transportation
  • Minimum of 1 year of clinical experience, preferably in long-term care

Preferred Qualifications:

  • Experience with patients with complex behavioral, social, or functional needs
  • Knowledge of geriatrics and chronic illness management
  • Proficient in electronic medical records documentation
  • Note: Employees must be in active regular status and remain in the role for at least 12 months to qualify for student loan repayment benefits, which require a 36-month role commitment within Home and Community.

The salary range for this role is $89,000 to $XXX,XXX annually, based on full-time employment and various factors such as location, experience, and certifications. Benefits include a comprehensive package, incentives, recognition programs, equity stock purchase, and retirement contributions.
