The Senior Community Care (SCC) program is a longitudinal, integrated care delivery program that coordinates the clinical care of members in a facility setting. The APC within SCC provides care to our highest-risk health plan members and is part of an interdisciplinary team. This position allows for work-life balance: no on-call, no weekends, and no holidays. It requires travel through the state of Connecticut.

The salary range for this role is $89,000 to $XXX,XXX annually, based on full-time employment and various factors such as location, experience, and certifications. Benefits include a comprehensive package, incentives, recognition programs, equity stock purchase, and retirement contributions.
