Job summary

CenterWell Senior Primary Care in Georgia is seeking a Nurse Practitioner or Physician Assistant to join a multidisciplinary team delivering comprehensive senior primary care. You will collaborate with physicians, pharmacists, care coaches, and behavioral health specialists to provide personalized, high-quality care for seniors.

We offer a team-based model with more time per patient, minimal on-call, generous PTO, and CME time.

Qualifications

  • Active NP or PA license in the state of practice.
  • Strong communication and interpersonal skills.
  • Familiarity with value-based care and integrated team-based models.

Responsibilities

  • Provide comprehensive care to seniors in accordance with clinical standards.
  • Collaborate daily with your care team to ensure coordinated, high-quality care.
  • Manage referrals, hospital/SNF coordination, DME, and home health services.
  • Participate in daily huddles and on-call rotation; support growth initiatives.
  • Document care accurately with support from quality-based coders.
  • Ensure compliance with licensing and accreditation requirements.

Your Role & Responsibilities
  • Provide comprehensive care to seniors in accordance with clinical standards.
  • Collaborate daily with your care team to ensure coordinated, high-quality care.
  • Manage referrals, hospital/SNF coordination, DME, and home health services.
  • Participate in daily huddles. CAs tend to lead the huddles
  • Document care accurately and efficiently with support from quality-based coders.
  • Participate in on-call rotation and support growth initiatives at your center.
  • Ensure compliance with all licensing and accreditation requirements.
What We’re Looking For
  • Master’s Degree in Nursing or completion of a PA program with board certification.
  • Active, unrestricted NP or PA license in the state of practice.
  • Minimum of 1 year NP or PA experience
  • Strong communication and interpersonal skills.
  • Passion for value-based care and integrated team-based models.
  • Basic computer proficiency (EMR, email, etc.).
Preferred

Use your skills to make an impact

  • Experience in value-based care, primary care or Medicare Advantage settings.
  • Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
  • Familiarity with HEDIS quality indicators and Medicare guidelines.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
