Schedule

Standard operating hours are 8:00 am – 5:00 pm; Monday-Friday. Specific work hours will be established through discussions with your supervisor.

Job Summary

Under the direct supervision of the Primary Care Physician and/or Medical Director, applies advanced knowledge/clinical skills necessary to assess/manage the healthcare of chronically ill geriatric participants in the LIFE program. Works directly under the Clinic Nurse Manager for daily operational issues.

Duties/Responsibilities

  • Practice within their scope, as defined by state laws, concerning physician oversight requirements, practice authority, and prescriptive authority.
  • Participate as part of the Interdisciplinary Team (IDT) and other required members.
  • Collaborate with PCP/Medical Director to deliver participant care for clinical outcomes, ensuring proper outcomes per LIFE-NWPA’s QAPI program.
  • Coordinate with Clinic Nurse Manager for operational and documentation issues including scheduling, charting, and procedural directions.
  • Perform health histories, physical exams, and patient teaching. Order, interpret, and follow up on lab tests and results.
  • Initiate emergency measures as necessary and recognize abnormalities for immediate referral.
  • Assess, initiate, and coordinate patient care daily under supervision.
  • Act as liaison with healthcare professionals, providing education, communication, and support.
  • Participate in 24-hour care delivery coordination and staff meetings.
  • Support Quality Improvement initiatives.
  • Other duties as assigned.

Required Skills/Abilities

  • Deal tactfully and patiently with personnel, participants, families, and the public.
  • Make independent decisions when needed.
  • Process and perform tasks for participant safety.
  • Attend ongoing educational programs; may include overnight travel.
  • Learn, teach, research, solve problems, and think analytically.
  • Organize work and follow through on tasks.
  • CPR and First Aid Certification.
  • Excellent written and verbal communication skills.

Education and Experience

  • Licensed as a Certified Nurse Practitioner in Pennsylvania.
  • One year of experience with the frail and elderly or willingness to undergo extensive training.

Requirements

  • Valid State driver’s license with a clean record.
  • Successful background check, physical, drug screening, TB screening.

Physical Requirements

  • Mobility throughout the workday.
  • Complete orientation to LIFE/PACE model, possibly involving overnight travel.
  • Ability to lift, carry, support participants.
  • Ability to drive a vehicle.
