Leoforce is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to provide comprehensive, patient-centered preventive care in Reading, PA. You will coordinate care across settings, prescribe medications, and educate patients while collaborating with physicians to optimize outcomes.

Job Description

Key Responsibilities

• Provide comprehensive, patient-centered preventive care, including routine physicals, health-risk assessments, immunizations, and screenings.
• Coordinate patient care across multiple healthcare settings and help ensure smooth transitions of care.
• Prescribe and manage medications, monitor effectiveness, and make adjustments based on patient needs.
• Develop and implement strategies focused on population health, disease prevention, health promotion, and chronic disease management.
• Collaborate with physicians and other healthcare professionals to develop, implement, and evaluate patient care plans.
• Educate patients and families on disease prevention, healthy lifestyle choices, and ongoing health maintenance.
• Utilize clinical data and population health strategies to improve patient outcomes.
• Participate in quality improvement and research initiatives designed to enhance patient care.
• Maintain accurate documentation within the Electronic Health Record (EHR) system.
• Effectively manage multiple patients, priorities, and responsibilities in a fast-paced clinical environment.
