Qualifications

• Current, unrestricted license as a Nurse Practitioner or Physician Assistant.
• Minimum of 3 years of experience as NP/PA.
• Demonstrated expertise in preventive care, care coordination, medication management/prescribing, and population health.
• Excellent communication and interpersonal skills; able to engage patients and work well in a team.

Responsibilities

• Providing high-quality preventive care services, including routine check-ups, risk assessments, immunizations, and screenings.
• Coordinating care across multiple settings and ensuring smooth transitions of care.
• Prescribing and managing medications, monitoring effectiveness, and adjusting as needed.
