Job summary

Jobot is seeking a Nurse Practitioner or Physician Assistant to join a community health center in Reading, PA. This on-site, permanent role focuses on preventive care, health-risk assessments, immunizations, and coordinating care across settings.

You will prescribe medications, participate in population health initiatives, and educate patients and families. The team is interdisciplinary, mission-driven, and committed to improving access to care for an underserved population.

Qualifications

• Current, unrestricted NP or PA license.
• 1+ years of NP/PA experience.
• Experience with preventive care, care coordination, medication management and patient education.

Responsibilities

• Provide comprehensive preventive care including routine physicals, risk assessments, immunizations and screenings.
• Coordinate patient care across multiple settings and ensure smooth transitions of care.
• Prescribe/manage medications and adjust therapies as needed.
• Develop strategies focused on population health and chronic disease management.
• Collaborate with physicians and other professionals to implement care plans.
• Educate patients and families on disease prevention and healthy living.
• Use EHR data to monitor outcomes and participate in quality improvement.

Job Details

• Provide comprehensive, patient-centered preventive care, including routine physicals, health-risk assessments, immunizations, and screenings.
• Coordinate patient care across multiple healthcare settings and help ensure smooth transitions of care.
• Prescribe and manage medications, monitor effectiveness, and make adjustments based on patient needs.
• Develop and implement strategies focused on population health, disease prevention, health promotion, and chronic disease management.
• Collaborate with physicians and other healthcare professionals to develop, implement, and evaluate patient care plans.
• Educate patients and families on disease prevention, healthy lifestyle choices, and ongoing health maintenance.
• Utilize clinical data and population health strategies to improve patient outcomes.
• Participate in quality improvement and research initiatives designed to enhance patient care.
• Maintain accurate documentation within the Electronic Health Record (EHR) system.
• Effectively manage multiple patients, priorities, and responsibilities in a fast‑paced clinical environment.

Experience & Qualifications:

• Current, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license.
• 1+ years of experience as a Nurse Practitioner or Physician Assistant.
• Demonstrated experience with:
  • Preventive care
  • Care coordination
  • Medication management and prescribing
  • Patient education
• Strong clinical decision-making, analytical, and problem‑solving skills.
• Strong commitment to patient-centered care and improving community health.
