Summit Healthcare Staffing is seeking an experienced Nurse Practitioner to join our client's expanding Multi-Speciality practice in Brooklyn, NY. This is an in-person position where you'll provide care through a telehealth platform in a modern, collaborative clinical environment.

Your expertise will help improve patient outcomes through comprehensive assessments, accurate diagnoses, and effective treatment plans.

Qualifications

• Active NP license in New York.
• Credentialed with Medicare and Medicaid.
• Excellent clinical assessment and communication skills.
• Telehealth experience is a plus.
• Ability to thrive in fast-paced environment.

Responsibilities

• Conduct telemedicine evaluations and follow-up visits.
• Diagnose, treat, and manage acute and chronic conditions.
• Develop and implement individualized treatment plans.
• Maintain accurate electronic medical records.
• Collaborate with physicians and clinical staff to ensure exceptional patient care.
• See approximately 25 patients per day.
