Job summary

Total Extended Care Services, a growing Physical Medicine and Rehabilitation group, seeks a Nurse Practitioner or Physician Assistant to join our PM&R team in sub-acute rehab settings.

In-person at skilled nursing facilities, with flexible full-time and/or part-time schedules. No on-call requirements; collaborative, interdisciplinary team focused on improving patient outcomes and reducing readmissions.

Qualifications

• Licensed NP in TN required.
• Certified Nurse Practitioner in TN required.
• Licensed Physician Assistant in TN required.
• Certified Physician Assistant in TN required.

Responsibilities

• Partner with the facility's therapy department to provide rehab care.
• Collaborate with the patient's primary care team to optimize outcomes.
• Timely charting/documentation, which can be done from home.

In this role, you will partner with our PM&R physician in the sub-acute rehab setting. This position is perfect for those passionate about improving patient outcomes and working within an interdisciplinary, specialized team.
