Job summary

MJHS is seeking a Nurse Practitioner for per diem coverage in Manhattan and the Bronx. You will provide and coordinate primary clinical care for high‑risk members, working with an interdisciplinary team to optimize outcomes during transitions from acute or sub‑acute care.

Required qualifications include a Master’s Degree in Nursing, NYS NP license, 1–2 years of NP experience with geriatric care, and a driver’s license. This is a per diem opportunity with community and facility sites across NYC.

Qualifications

• Master’s Degree in Nursing from an accredited Nurse Practitioner program.
• One to two years of NP clinical experience required.
• Minimum one year in geriatric specialty.
• NYS NP License/Registration and DEA certificates required.

Responsibilities

Job description

Per Diem Rates

• Telehealth visits for the community = $150 (average compensation)
• In person visits = $150–$210 (average $180 FFS)

Orientation hourly rate: $90/Hour

This field position will cover Manhattan and Bronx regions.

Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPNs who provide care to Elderplan members who are residents of assisted living and long‑term care facilities, as well as to those living at home.

Responsibilities

• Provide and/or coordinate primary clinical care services for assigned high‑risk members and facilitate supportive care during transitions from acute or sub‑acute care settings.
• Act as team player, member’s advocate, and clinical lead while working closely with the interdisciplinary team to coordinate the member’s care and optimize health outcomes.
• Serve as a resource and contact for the member/designated representative regarding all aspects of the individualized care plan and disease process.

Qualifications

• Master’s Degree in Nursing, graduate of an accredited Nurse Practitioner program required.
• One to two years of clinical experience in the role of nurse practitioner required.
• Minimum one year in geriatric specialty.
• Previous clinical RN experience in acute, sub‑acute and/or long‑term care setting preferred.
• Prior experience in managed care a plus.
• NYS NP License, Registration, DEA certificates.
• Driver license and car required.
