Job summary

MJHS is seeking a Nurse Practitioner for per diem coverage in Manhattan and the Bronx. You will provide and coordinate primary clinical care for high‑risk members, working with an interdisciplinary team to optimize outcomes during transitions from acute or sub‑acute care.

Required qualifications include a Master’s Degree in Nursing, NYS NP license, 1–2 years of NP experience with geriatric care, and a driver’s license. This is a per diem opportunity with community and facility sites across NYC.

Qualifications

• Master’s Degree in Nursing from an accredited Nurse Practitioner program.
• One to two years of NP clinical experience required.
• Minimum one year in geriatric specialty.
• NYS NP License/Registration and DEA certificates required.

Responsibilities

• Provide and/or coordinate primary clinical care for high‑risk members and facilitate transitions from acute or sub‑acute care.
• Collaborate with the interdisciplinary team to coordinate care and optimize outcomes.
• Serve as a resource for members and designated representatives regarding care plans and disease processes.
