Job summary

MJHS Medical Associates, P.C. seeks a Nurse Practitioner to deliver and coordinate primary care for Elderplan members in assisted living and long-term care facilities in Nassau and Suffolk Counties. You will develop care plans and ensure preventive care in alignment with health outcomes.

You will act as a clinical resource to providers and facilities, coordinating care for residents and serving patients at home when needed. A $15,000 sign-on bonus or student loan assistance is offered.

Qualifications

• Master's Degree in Nursing from an accredited program.
• NYS NP License required.
• DEA certificate required.
• 1–2 years clinical experience as NP required.
• Minimum 1 year geriatric experience.
• Previous RN experience in acute/subacute/long-term care preferred.

Responsibilities

• Develop care planning and ensure access to preventive care goals.
• Provide and coordinate primary clinical care for assigned residents in long-term care facilities.
• Serve as clinical resource to participating providers and facilities.

This position will cover Elderplan members who are residents of assisted living and long term care facilities in Nassau and Suffolk Counties.

MJHS is a large not-for-profit health system in the Greater New York area. We also offer Elderplan/HomeFirst: health plans for Medicare and dual-eligible individuals. Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPN's who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.

In collaboration with physician, you will provide and coordinate primary care services to an assigned panel of Advantage members who reside in participating long term care facilities.

• You will insure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives
• Provide and/or coordinate primary clinical care services for assigned Nursing Home Residents
• You will act as a clinical resource to the participating providers and facilities
• You will act as a resource and contact for member/designated representative regarding all aspects of member individualized care plan and disease process

• Masters Degree in Nursing, graduate of accredited Nurse Practitioner program required
• One to two years clinical experience in role of nurse practitioner required
• Minimum one year in geriatric specialty
• Previous clinical RN experience in acute, subacute and/or long term care setting preferred
• Prior experience in manage care a plus
• NYS NP License required
• DEA certificate required
