Job summary

Visionary Healthcare Partners is seeking a Part-time Per Diem Nurse Practitioner in the Greater Los Angeles area to provide annual wellness and high‑risk care for Medicare patients. You will see patients in clinics and their homes, with telehealth as appropriate.

Responsibilities include comprehensive wellness visits, medical histories, medication reconciliation, tests, and patient education, while maintaining HIPAA compliance and accurate EMR documentation.

Qualifications

• Nurse Practitioner certification through the ANCC or AANP.
• Two years NP experience in primary care or related specialty preferred.
• Active CPR/BLS certification and DEA eligibility after hire.
• Ability to travel throughout the Greater Los Angeles area.

As a Per Diem Nurse Practitioner, you will provide annual wellness and high-risk care to Medicare patients throughout the Greater Los Angeles area. You will primarily see patients in clinics and their homes, with telehealth visits scheduled when appropriate.

You will conduct comprehensive wellness visits, document medical histories and symptoms, reconcile medications, and perform appropriate tests and screenings. You will also educate patients about preventive care and prescribed treatments, maintain accurate medical records, and collaborate with primary care clinicians and other team members to deliver coordinated, holistic care.

You will complete documentation promptly through electronic medical records and clinical portals while protecting patient confidentiality and complying with all HIPAA requirements.

This per diem position requires flexibility across coverage areas. The organization generally schedules home visits within 35 miles of your home region, but you may travel to other parts of the Greater Los Angeles area when coverage or scheduling needs change.

Requirements

• Nurse Practitioner certification through the ANCC or AANP
  
• Two years of Nurse Practitioner experience in primary care, family medicine, internal medicine, gerontology, palliative care, or a related specialty preferred
  
• Knowledge of chronic-care management and medication treatment options
  
• Active CPR/BLS certification
  
• Ability to obtain and maintain DEA registration and prescriptive authority after hire
  
• Ability to travel throughout the Greater Los Angeles area
  

Home-based care experience and English-Spanish bilingual skills are preferred.

Competitive compensation ($69.71-$76.92/hourly)
