Position Overview

• 2+ years of experience.

• Family Nurse Practitioner (FNP).

Full-time preferred. Part-time considered. Outpatient. No hospital rounds. No overnight calls.

Responsibilities

• Provide comprehensive primary care to adult and geriatric patients.
• Manage chronic conditions (HTN, DM, CHF, COPD, etc.).
• Conduct annual wellness visits.
• Meet quality metrics aligned with HEDIS/value-based contracts.
• Collaborate with supervising physician and care team.
• Document efficiently in EMR.
