Job summary

Prudential MD is recruiting a Per Diem/Part-Time Nurse Practitioner or Physician Assistant to provide in-home health evaluations across Los Angeles and Orange County. Visits last about 45 minutes, focusing on medication review, medical history, and physical exams.

You will travel locally, engage with geriatric patients in homes and senior communities, and build meaningful relationships to improve health outcomes, while maintaining documentation and coordinating with the care team.

Qualifications

• Active NP or PA license in California.
• Valid driver’s license and reliable transportation.
• Experience with geriatric or chronic care populations preferred.
• Strong communication and interpersonal skills.
• Ability to work independently.

We are currently seeking a Per Diem/Part-Time Nurse Practitioner or Physician Assistant to join our mobile care team. In this role, you will provide in-home health evaluations, build meaningful relationships with patients, and help bridge gaps in care.

Role Overview

The typical visit lasts about 45 minutes, allowing time to connect one-on-one, review medications and medical history, answer questions, and complete a physical evaluation.
