American Physiatry in Pennsylvania is seeking a compassionate Nurse Practitioner to join our team and work closely with physical therapy to focus on pain control and functional rehabilitation in skilled nursing facilities.

The ideal candidate is licensed in PA, experienced in independent caseload management, and comfortable documenting thoroughly while collaborating with a physiatrist and health-care colleagues to optimize patient outcomes and quality of life.

Qualifications

• Licensed Nurse Practitioner with PA certification in PA.
• Experience working independently with a caseload.
• Prompt, thorough clinical documentation in compliance with policies.

Responsibilities

• Conduct thorough assessments of patients, identifying and managing medical conditions under supervision of a Physiatrist.
• Develop and implement individualized care plans focusing on function and rehabilitation to improve quality of life.
• Collaborate with the healthcare team to ensure comprehensive care, including medication management and therapy recommendations.
• Educate patients and families on treatment plans, health maintenance, and preventive care.
• Monitor and evaluate patient progress, adjusting care plans as necessary to achieve optimal outcomes.
