American Physiatry in Virginia is seeking a compassionate Nurse Practitioner to join our healthcare team. Qualifications

• Master's degree in Nursing is required.
• Virginia NP license is required.
• 2+ years in skilled nursing facilities preferred.
• Experience documenting care plans and regulatory compliance.

Responsibilities

• Conduct comprehensive patient assessments under the supervision of a Physiatrist.
• Develop and implement individualized care plans focusing on function and rehabilitation.
• Collaborate with the healthcare team for medication management and therapy recommendations.
• Educate patients and families on treatment plans and preventive care.
• Monitor patient progress and adjust care plans to achieve outcomes.

Key Responsibilities

• Conduct thorough assessments of patients, identifying and managing medical conditions under the supervision of a Physiatrist.

• Develop and implement individualized care plans, focusing on function and rehabilitation to improve patients' quality of life.

• Collaborate with the healthcare team to ensure comprehensive care, including medication management and therapy recommendations.

• Educate patients and their families on treatment plans, health maintenance, and preventive care.

• Monitor and evaluate patient progress, adjusting care plans as necessary to achieve optimal outcomes.

Qualifications

• Licensed Nurse Practitioner with a valid Virginia certification.

• Experience working independently while effectively managing your own caseload.

• Submit accurate and thorough clinical documentation promptly, ensuring compliance with company policies and regulatory requirements.
