Job summary

American Physiatry is seeking a compassionate Nurse Practitioner to join our team in skilled nursing facilities, focusing on pain control, function, and rehabilitation. You will work under a Physiatrist to assess patients and develop individualized care plans that improve quality of life.

You will collaborate with the healthcare team on medications and therapy recommendations, educate patients and families, and monitor progress to adjust plans for optimal outcomes.

Qualifications

• Master's degree required.
• Experience in Skilled Nursing Facility preferred.
• Licensed Nurse Practitioner with current license.

Responsibilities

• Conduct comprehensive patient assessments under supervision of a Physiatrist.
• Develop and implement individualized care plans focusing on function and rehabilitation.
• Collaborate with the healthcare team on medications and therapy recommendations.
• Educate patients and families on treatment plans, health maintenance and preventive care.
• Monitor patient progress and adjust care plans to achieve optimal outcomes.

Key Responsibilities

• Conduct thorough assessments of patients, identifying and managing medical conditions under the supervision of a Physiatrist.

• Develop and implement individualized care plans, focusing on function and rehabilitation to improve patients' quality of life.

• Collaborate with the healthcare team to ensure comprehensive care, including medication management and therapy recommendations.

Qualifications

• Licensed Nurse Practitioner with a valid New Jersey certification.

• Experience working independently while effectively managing your own caseload.

• Submit accurate and thorough clinical documentation promptly, ensuring compliance with company policies and regulatory requirements.
