Job summary

Main Line Health in Delaware and Montgomery Counties is seeking a Nurse Practitioner for Palliative Home Care to provide independent symptom management and goals-of-care discussions in patients' homes.

The role emphasizes collaboration with physicians, patient-centered support, and ongoing professional development; requires MSN and PA license in Pennsylvania, BLS, ACLS, and credentialing with MLH.

Qualifications

• 2 years Nurse Practitioner experience preferred in palliative home care.
• Minimum of 2 years Nursing experience in palliative home care preferred.
• Completion of a Nurse Practitioner program required.
• MSN required.
• Licensed by the Pennsylvania State Board of Nursing Examiners as a CRNP.
• Provider BLS required, ACLS certification preferred.
• PALS may be required.
• Successful MLH credentialing process completion.

Responsibilities

Previous home care, hospice, or palliative care experience is preferred.

Position

Nurse Practitioner, Palliative Home Care

Shift

Full Time, 40 hours/week, Day Shift. No Holidays. On call rotation weekends.

Experience

2 years Nurse Practitioner experience preferred in palliative home care.

Minimum of 2 years Nursing experience in palliative home care preferred.

Education

Completion of a Nurse Practitioner program required.

MSN Required.

Licensures/Certifications

Licensed by the Pennsylvania State Board of Nursing Examiners as a Certified Registered Nurse Practitioner.

Provider BLS (American Heart Assoc/American Red Cross) required.

Require ACLS certification.

May require PALS (pediatric advanced life support).

Successful completion of the MLH credentialing process through the Medical Staff office, with temporary or permanent privileges approved.
